I've got one and yes they do.
You do have ot use it regularly but every other day to start with then every day for five minutes - surprisingly effect way of coaxing more out of your body - basically feels like your building up the intercostal muscles enabling the user to breathe in more air = more oxygen to your muscles.
Used it prior to running a 10k race and took four minutes off my previous best - not competitive runner at all and other than the powerbreathe done no more training than any other time so must have helped even if it was just mentally!!!

Used to use one during the warm up before our morning sessions, then ditched that idea and started using whilst driving to the gym using a bit more resistance.
A lot of people boubt them, but as far as i'm aware the Marines use them in some of their training, and if it's good enough for them...
