I'd more or less begin at the beginning again after that sort of lay off. A well thought out 4 day split to get you back into it then perhaps 3 months later moving to 5x5. For the 4 day split include the major compounds - Bench, Deadlifts, Squats, Dips, Etc. Draft something up and we'll be happy to look at it for you.
