If you have a legitimate low Testosterone blood result either via an NHS endocrinologist or an online source such as Medichecks will you be able to prescribe a TRT dose and if so how would the Test come ie gel, Nebido or Sust etc?

Yes, we are certainly looking into doing that. We just have to make sure the testosterone readings are legitimate so our doctors are happy prescribing.

The options we are looking at right now would be Tostran Gel (2% testosterone), Nebido (1g/4ml testosterone undecanoate), or Restandol 40mg Testocaps.

I can't give you exact prices but Nebido is not cheap unfortunately. The most cost effective are Restandol followed by the Tostran gel.

If someone with low testosterone was looking to adopt a TRT regime would your doctors give a treatment plan?

If so, can you tell us how they would work out the ideal protocol for that particular person? The reason I ask is most doctors and endocrinologist are absolutely clueless when it comes to testosterone replacement therapy and usually end up doing more harm than good in the short term.

I think I'll be able to better answer this.

You're absolutely right about finding a specialist endo that has a good grasp of male TRT. It's not easy and we're trying to get one on board now. The difficulty lies in the initial diagnosis of hypogonadism, it should be done by an experienced endocrinologist. Unfortunately male TRT still has a negative stigma, especially in the NHS. Most will suspect pituitary issues at first instance (because thats more common in their patient groups) and will want to rule that out. Some endos will attempt therapy with clomiphene or hCG (esp for younger patients) before committing to TRT.

The protocol itself isn't difficult. Once the initial diagnosis is made one of our GPs are more than qualified to issue further prescriptions and adjust doses. It's basically a very slow titration up (usually testosterone gel) until symptoms improve, retest then maintenance.

I will make a list of the members interested and keep you updated on this, as like I said it's in the pipeline as we speak. We just have to make sure it's all safe and legal.

I'm afraid not unfortunately. As an active chest infection can turn serious quite quickly and is considered acute it isn't something we can treat online. I also wouldn't want us sending you antibiotics and you not receiving them in time because Royal Mail was delayed for instance. (rare but can happen)

If you need some now I would suggest calling 111 and getting the on call doctor to fax a prescription to a local pharmacy. It used to be common practice in our area, as we can access patient data on the SCR and check for recurrent infections.
